For the past week or so I noticed that my remote control (clicking the microphone) was not working. I use this once in a blue moon, so I had no idea of what it could be and my phone has suffered no trauma recently. (Other than an uneventful upgrade to 2.2.1!)
So having the opportunity, I made an appointment to see a Genius in the London Regent Street store today. After stating that they would be happy to replace components (headphones, iPhone) the Genius first poked the socket with a bent paper clip and pulled out paper or fluff. After that it worked perfectly! He told me that one can do this at home but obviously with care, and not to stab blindly!
I'm impressed, and not having found any references to the problem when I searched for it I thought I would offer this experience. Naturally I have to warn that you would do such a thing at your own risk, it was not formally endorsed by Apple as a solution, and your mileage may vary!
